These forms may be used for the purpose of obtaining a default and default judgment under Federal Rule of Civil Procedure 55. The procedure under Rule 55 is a two-step process:
(1) entry of default by the Clerk and (2) entry of judgment by default, by the Clerk when the claim is for a sum certain; by the Court in all other instances.

Step One: File the Request for Entry of Default, with the Affidavit in Support of Request for Entry of Default as an attachment. E-mail the proposed Entry of Default, in WordPerfect format, to the appropriate Clerk’s Office e-mail address:

Step Two: After the Clerk has entered the Default, file the Motion for Default Judgment with the Affidavit in Support of Motion for Default Judgment as an attachment. E-mail the proposed Default Judgment in WordPerfect format to the appropriate Clerk’s Office e-mail address.
